"Panic Attacks, Heart Palpitations, and Elevated Cortisol"

Presenting History

When Dana first presented for care, she reported six months of escalating anxiety and panic attacks consistent with Adrenal Fatigue and stress-response dysfunction. 

Her history included a milder episode of adrenal dysfunction years earlier that had resolved. However, after cumulative life stress — including the loss of her daughter — her symptoms returned with greater intensity. The episodes appeared to occur “out of nowhere.”

“I would be fine and then panic attacks would just hit me.”

Because of the severity and unpredictability of her symptoms, she feared a neurological disorder. Cardiology workup was structurally normal. Cortisol testing, however, revealed elevated levels consistent with a stress-response system under sustained strain.

Despite identifying adrenal involvement, previous care had focused primarily on symptom management and compounded supplements. After six months, she described her condition this way:

“I was making it, but I was not improving.”

Initial Clinical Assessment

At intake, Dana’s presentation reflected classic stress-response dysregulation:

  • Hyperarousal with intermittent panic

  • Sleep disruption

  • Elevated cortisol

  • Low stress tolerance

  • Post-stress crashes

  • Ongoing sympathetic dominance

We approached her case as systemic stress-response dysfunction, focusing on recalibrating her entire stress-response system while easing symptom burden and restoring stability.

Treatment Strategy

We implemented a structured, step-wise plan designed to reduce physiological load, stabilize blood sugar and electrolytes, decrease sympathetic overactivation, increase parasympathetic tone, and gradually restore functional capacity.

“If I had a bad reaction, Dr. Neville said, just don’t do that. Take it off the table.”

This included pacing daily activity to prevent crashes, gradual supplement introduction with reaction monitoring, removal of aggravating agents, regular virtual consultations, and ongoing office hours for accountability and recalibration.

The emphasis was stability first — not intensity.

Course of Recovery

Recovery followed the typical non-linear arc seen in stress-response dysfunction. There were periods of regression followed by measurable gains.

The first meaningful turning point came when she noticed something subtle but profound:

“Then one day, I didn’t have any panic attacks at work.”

From that point forward, panic frequency declined, palpitations diminished, sleep began to normalize, and stress tolerance improved.

Dana tracked her progress visually using a calendar system marking severe, moderate, and stable days. What began as predominantly red gradually shifted toward predominantly green over the course of several months.

Nearly one year into structured care, her physiology had stabilized. She no longer scans her body for symptoms or fears sudden collapse. She responds to stress without physiological shutdown. Functional resilience returned, and daily life no longer required constant monitoring.

Clinical Interpretation

Dana’s case illustrates a common pattern in Adrenal Fatigue and stress-response dysfunction:

  • Normal structural testing

  • Elevated cortisol

  • Panic symptoms misinterpreted as primary psychiatric disorder

  • Stress-response system operating beyond capacity

By addressing stress-response physiology rather than suppressing symptoms, her nervous system recalibrated and functional resilience returned.
